11. As a resuLt of the aforesaid coL Lision, the
Plaintiff, Marie Fisher, has suEfered serious and permanent
injuries, incLuding, but not limited to the following:
(a) severe cervical and
thoracic myofascitis, with
associated soft tissue
in juries;
(b) protrusion of the
intervertebral disc at the
1.5-51 level;
(c) contusion of the right and
left knees;
Id) right C6-C7 cervical
radiculopathy;
(e) lower lumbar
['ad iculopathies;
(f) myofascial pain syndrome,
right neck and shouldar and
right flank;
(g) severe strain and sprain of
the muscles. tendons.
ligaments and other soft
tissues at or about the
cervical spine;
(h) severe strain and sprain of
the muscles, tendons,
ligaments and other soft
tissues at or about the
lumbar spine;
(i) severe abdominal wall
contusion;
(j) torn rectus abdominous
muscle;
(k) bulging cervical disks at
the C5-C6 level
(II aggravation of previously
asymptomatic degenerative
disc disease at or about
