RYAN BENDER, Defendant CIVIL ACTION - LAW IN CUSTODY ORDER OF COURT AND NOW, this day of January, 2004, upon consideration of the attached Custody Conciliation Summary Report, it is hereby ordered and directed as follows: 1. This Court's Order of November 27, 2002 shall remain in full force and effect with the following modifications. 2. No later than February 13, 2004, the minor child will have had a physical examination and the immunizations required by Pennsylvania law for school attendance and recommended by a physician, will be up-to-date. 3. Father shall be strictly responsible for the timeliness of the child's attendance at school on school days when the child has been at his custody the evening before. In the event that the child is ill and unable to attend school, a doctor's note will be provided indicating the diagnosis and recommendation for school absence, should her condition warrant an absence. 4. The parties shall contact Parent Works or participate in a parenting education program in order to improve their ability to communicate together regarding parenting issues and work cooperatively in the child's best interest. 5. The Custody Conciliation Conference shall reconvene on February 17, 2004, at 11:30 a.m. at the office of the Custody Conciliator, Melissa Peel Greevy, Esquire, 301 Market Street, Lemoyne, PA 17043. At the time the Conference reconvenes, it is anticipated that the parties will provide specific and detailed information regarding the progress on obtaining the child's medical care, immunizations and improvement in the child's school attendance. The parties will also provide a status report regarding their ongoing participation in parenting education. In the event that significant improvement is not made in these areas of concern, the parties are advised that significant changes to the custodial schedule may be warranted. BY TH O RT. (??// Gl J.
