Gladwin, her husband, and their daughter, Tracy. 6. The relationship of Defendant Misty Jo Hoffman to the child is that of mother. She last resided with the following persons: A friend known to P~aintiffs only as Bonnie. 7. The relationship of Defendant Brad M. Newberry to the 'child is that of father. He currently resides with the following persons: Unknown. 8. The Plaintiffs have not participated as a party or witness, or in any other capacity, in other litigation concerning the custody of the child in this or another court. Plaintiffs have no information of a custody proceeding concerning the child pending in a court of this Commonwealth. Plaintiffs do not know of a person not a party to the proceedings who has physical custody of the child or claims to have custody or visitation rights with respect to the child. 9.
