Segaration and Progerty Settlement Agreement
This Agreement, made this./' t,( day of July, 199!i betwef:ln
William E. Samuels, of Camp Hill, York County, Pennsylvania,
hereinafter called "Husband" and Suzanne Samuels, of Camp Hill.
York County, Pennsylvania. hereinafter called "Wife.
The parties hereto, being Husband and Wife, were lawfully
married on June 9, 1984, at Harrisburg, Pennsylvania.
Differences have arisen between Husband and Wife in
consequence of which they intend to e~d their marriage and live
aparl from each other.
Husband and Wife desire to settl6 and determine finally dnd
for all time. their mutual property rights.
Neither Husband nor Wife shall contract or incur any debt or
liability for which the other or their property or estate might be
responsible.
The parties hereby release and discharge absolutely and
forever each other from any and all rights, claims and demands,
past, present and future specifically from the following: support
from the other, division of property, claims or rights of dow~r,
rights to any and all assets held in the name of the other (such as
Real Estate. Bank Accounts. Individual Retirement Accounts. Mutual
Funds, Retirement Benefits, etc.), right to act as eKecutor or
administrator in the other's estate, rights as devisee or legatee
in the Last Will and Testament of the other, any claim or right as
beneficiary in any life insurance policy of the other, any claim or
right in the other parties estate.
It is the intent of th~ parties to be legally bound hereb~ and
this Agreement shall bind the parties hereto and their respective
heirs, eKecutors, administrators, and assigns.
This Agreement shall be governed, construed and enforced under
the statute and case law of the Commonwealth of Pennsylvania.
IN WI~NESS WHEREOF, the parties have eKecuted this Agreement
the day and year first written above.

NOTICE TO DEFEND AND CLAIM RIGHTS
You have been sued in court. If you wish to defend against
the claims set forth in the following pages, you must take prompt
action. You are warned that if you fail to do so, the case may
proceed without you and a decree of divorce or annulment may be
!entered against you by the Court. A judgment may also be entered
lagainst you for any other claim or relief requested in these
ipapers by the Plaintiff. You may lose money or property or other
rights important to you, including custody or visitation of your
children.
When the ground for divorce is indignities or irretrievable
breakdown of the marriage, you may request that the court require
you and your spouse to attend marriage counseling prior to a
divorce decree being handed down by the court. A list of
marriage counselors is available in the Office of the
Prothonotary at the Cumberland County Court House, Carlisle. You
are advised that this list is kept as a convenience to you and
you are not bound to choose a counselor from the list. All
necessary arrangements and the cost of counseling sessions are to
be borne by you and your spouse.
IF YOU DO NOT FILE A CLAIM FOR ALIMONY, DIVISION OF
PROPERTY, LAWYER'S FEES OR EXPENSES BEFORE A DIVORCE OR ANNULMENT
IS GRANTED, YOU MAY LOSE THE RIGHT TO CLAIM ANY OF THEM.
YOU SHOULD TAKE THIS PAPER TO YOUR LAWYER AT ONCE. IF YOU
DO NOT HAVE A LAWYER OR CANNOT AFFORD ONE, GO TO OR TELEPHONE THE

6. Neither party is a member of the armed forces of the
United States of America.
7. The Plaintiff avers as the grounds upon which this
action is based is that the marriage between the parties hereto
is irretrievably broken.
8. The Plaintiff has been advised that counseling is
available and that Plaintiff may have the right to request that
the Court require the parties to participate in counseling.
9. The Plaintiff requests the Court to enter a decree of
divorce.
WHEREFORE, Plaintiff Suzanne V. Samuels requests this
Honorable Court to enter a Decree in Divorce, divorcing the
Plaintiff from the bonds of matrimony heretofore existing between
the Plaintiff and the Defendant.

SUZANNE V. SAMUELS. being duly sworn according to law,
deposes and says:
1, I have been advised of the availability of marriage
counseling and understand that I may request that the cou~t
require that my spouse and I participate in counseling.
2. I understand that the court maintains a list of marriage
counselors in the Office of the Prothonotary, which list is
available to me upon request.
3. Being so advised, I do HQI request that the court
require my spouse and I participate in counseling prior to a
divorce decree being handed down by the court,
I understand that false statements herein are made subject
to the penalties of 18 Pa. C.S. S 4904 relating to unsworn
falsification to authorities.
