2. Dclendant is Gary John Chambers, an adult individual, currenlly rcsiding at 8466
Main Road, Jamesport. NY 11947
3. Plaintilf seeks custody of the child, Michael John Chambers, who was born on July
16, 1984.
The child was not born out of wedlock.
Since Ihe child's birth, the child has residcd wilh the following persons at Ihe following
addresses for the rollowing periods or time:
The child has rcsided with Plainlilf since his birth, and since Oclober 7, 1994, wilh PlainlitT
and his stepfalher at Plaintitl's current address.
The natural mother or the child is Plaintill' who resides as aforesaid, She is married.
The natural lather of the child is Gary John Chambcrs who resides as aforesaid. He is single
and living separately.
