ORDER
As a result of the Departmental hearing which was held on 10-20-95 and
02-20-96, your Certificate of Appointment as an official safety in-
spection station is suspended, under Section 4724(a) of the Vehicle
Code, as amended. Consequently, no safety inspections may be per-
formed at your etation during the euspension. Your suspeneion will be
for a total of three (3) years effective 04-05-961 see Attachment A
for details.

You are ordered to surrender to the bearer of this notice, who
authorized representative of the Department of Traneportation,
certificate(s) of Appointment for Official Inspection Station
and all safety inspection stickers.
One (1) month prior to the expiration of your suspension, you may sub-
mit an application for reappointment.
You have a right to appeal this Departmental order to the Court of
Common Pleas in the county in which the above referenced inspection
station is lccated within thirty (30) days of the date of this notice.
A signed, certified, time-stamped copy of the appeal and a supersedeas
must be served upon the Department at the address below in order for
your privileges to be restored pending appeal.

ORDER
AND NOW, this l. ~ It, day of ~, 1996, the Court accepts the
Stipulation of Agreement submitted by parties in the above captioned matter.
Wherefore, the certificate of appointment ofTwin Wheels Automotive as on official
inspection station is suspended for a period of eight months (three, two-month suspensions for
faulty inspection and one, two-month suspension for improper record keeping; said suspensions ore
to nm consecutively for a total suspension of eight months), The parties shall comply with the
terms and conditions as set forth in the attached Stipulation of Agreement which is hereby
incorporated by reference.

9, On May 24, 1996, the Station was restored pending appeal.
10, The Department has agreed that the Station experienced extenuating
circumstances due to medical problems and overly relicd upon its mechanic Robert B, Manning.
11, The Department has agreed to reduce 011 suspensions specifically stated above to
three, two-month suspensions for faulty inspection and one, two-month suspension for improper
record keeping, Said suspensions are to run consecutively for a total suspension of eight months.
12, In exchange for the reduccd suspensions, Petitioner Station has agreed to
withdraw all appeals and to abandon its attempt to avoid the suspension (by establishing another
official inspection station on the same premises), Petitioner Station agrees not to maintain an
official inspection station on the same premises during the period of the eight-month suspension,
and any other application submitted will be reviewed in accordance with departmental
regulations,
13. Petitioner and Respondent agrcc to abide by all of the foregoing terms and
conditions,
WHEREFORE, for the above reasons, Petitioner and Respondent respectfully requests
this Honorable Court to accept this Stipulation of Agreemcnt and enter upon the rccord the
attached proposed order,
