13. The minor child ha. had no other contact with the father
whatsoever,
14. The minor chi 1 d has had no contact with her paternal
grandparents.
15. The PetitIoner. mother desires to have the minor child's
name changed because Petitioner no longer uses her maiden name of
FHzkee and would like the child to have the same last name as her
mother.
16. Petitioner believes that it is in the best inter.estl of
the minor chi ld that her. name be changed to "POX" so that she wi 11
continue to have the same last name as her Mother.
17, The father has never exercised any parental rights in
reference to the chi 1 d and had to be compell ed by court to pay
child support for the child.
18, The child haB never been known by the father'. name and
Petitioner. Mother believes it would be best for the ohild to
change her last name to her natural mother's married last name of'
Pox.

6. Petitioner, mother married Christopher D. Pox on
September 30, 1994.
7. The father is ourrently married.
8. ANDEE NOELLE FITZKEE is five (5) years old, having been
born in Dauphin County, Pennsylvania on December 14, 1990. A copy
of the certificate of birth is attached hereto, made a part hereof
and marked as Exhibi t "A".
9. ANDE! NOELLE rITZIER resides with her mother and
stepfather, Christopher D. Pox, at 56 Ashford Drive, Inola,
Cumberland County, Pennsylvania.
10. Petitioner's maiden name is Fitzkee and she was unmarried
at the time of the birth of her daughter, Andee Noelle Fitzkee.
11. The minor child has always resided with Petitioner, who
has primary physical and legal custody of the child.

12. The onl y time the minor chil d saw her father. Jeff rey A.
Stafford was in open court at the paternity trial.
