1. Appellant, Glenn T. Marohand, is an adult individual whom
currently resides at 504 Poroha Terrace, Camp Hill, Cumberland
County, Pennsylvania.
2. Appellee, Commonwealth of Pennsylvania, Pennsylvania
Department of Transportation, Bureau of Driver Licensing,
Harrisburg, Dauphin County, Pennsylvania, 17123, is a state
agency.
3. Your Appellant was arrestod for allegedly operating his
! motor vehicle while driving under the ,influence on February 1,

" 1996, at 0153 hours by Patrolman, Jeffrey O'Donnell, of the West
"
Shore Regional Polioe Department, Lemoyne, Pennsylvania.

4. Your Appellant was arrested for violations which occurred
I on Poplar Church Road, Old Orchard Road, Yverdon Drive, and West
, Foxcroft and Kevin Court, Camp Hill, Cumberland County, PA.
5. Your Appellant was informed that he was going to be given
a blood testing to determine his blood alcohol content,
6. Your Appellant was notified on or about March 25, 1996,
that his license was being suspended for a period of one year
which would go into effect April 25, 1996 at 12:01 a.m. A true
and correct copy of the Notice is attached at Exhibit "A".
