Now comes Joseph Smith, pro se, who deposes and says he is the plaintiff
here1n. Plaintiff respectfully submits this complaint in divorce.
1. Plaintiff currently resides at S,C. I. Camp Hill, 2S00 Lfsburn Road,
P.O. Box 200, Camp Hill, Cumberland County, Pennsylvania, since April 2, 1992.
2. Defendant is Joan Smith, who currently resides at 2911 Wilder Street,
Philadelphia, Philadelphia County, Pennsylvania, since August 1983.
3. Plaintiff has been a bona fide resident of the Commonwealth of
Pennsylvania for at least six (6) months immediately prior to the filing of
this complaint.
4. Plaintiff and defendant were married on December 11, 1974, at
Tinicum, Pennsylvania,
S, Plaintiff alleges the marriage between the parties is irretrievably
broken as the grounds on which this action is based.
6. Plaintiff has been advised that counseling is available and that
he may have the right to request that the court require the parties to
participate in counseling.
7. There have been no prior actions of divorce or for annulment between
the parties.

To the Honorable Judge presiding of thp. above said Court:
Now c~mes Joseph Smith, pro se, who deposes and says he is the petitioner
herein. Petitioner respectfully submits this petition to proceed in forma
pauperis. In support thereof:
1. This petition to proceed in forma pauperis is made pursuant to the
authority contained in Pa.R.Civ.P., Rule 1920.62, relating to proceedings:~y
indigent parties.
2. Petitioner avers because of his financial condition he is unable
to pay the fees and costs of proceeding in an action in d1vorce.
3. Petitioner is unable to obtain funds from anyone, including family
and associates, to pay the fees and costs of proceeding in this matter.
4. Petitioner currently receives approximately sixty-five dollars
($6S.00) a month as nominal prison wages.
S. Petitioner has not received any income from any other source for
a periOd of in excess of twelve (12) months.
6. Petitioner is truely indigent and owns no properties, securities,
or monies (including savings accounts) to pay the fees and costs of proceeding
in this matter.
