AND NOW, this '7t~day of May, 1996, upon consideration of the
Petition For Change of Name Pursuant To 54 Pa. C.S. 5701 et seq.,
it is ORDERED that a hearing on Petitioner's request be held on
Friday, JUlie 28, 1996, at 11.00 a.m., in Courtroom No.5,
Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that the notice of the filing of said
Petition and the aforesaid date, time and place of the hearing be
published once in the Cumberland Law Journal and a newspaper of
general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other
requirements of C.C.R.P. 5, and the Act of December 16, 1982, P.L.
1309, 56, and 54 Pa. C.S. 55701 et seq. (1995 Supp.).

4. Mr. Stevens is not Crystal's father, and at no time has petitioner held out Crystal
to be the daughter of Mr. Stevens.
S, Crystal's biological father is Doyle Leo Wilborn. A true and correct copy of the
hospital's informational certificate is attached as Exhibit B.
6. After Crystal's birth, petitioner and Mr. Wilborn were married on March 11,
1988. Subsequently, petitioner and Mr. Wilborn were divorced on June 20, 1991.
7. Mr. Wilborn currently pays child support through the Cumberland County
Domestic Relations Office for the support of Crystal. The original Support Order was entered
in Dade County Georgia on October 2, 1992. Since Crystal.'s birth, petitioner has moved from
Georgia to York County and then to Cumberland County where she currently resides. A
certified copy of the Transfer Order from York County to Cumberland County is attached as
Exhibit C as evidence that Mr. Wilborn has been judicially determined to be the father of
Crystal.
8. Mr. Wilborn has consented to Crystal's name change from Stevens to Wilborn.
Mr. Wilborn's consent to name change is attached as Exhibit D.
9. Mr. Stevens is not an intr.rested party and has not been given notice of this
proceedi.lg because:
A. He is not Crystal's father;
B. He is unaware of Crystal's existence and therefore has not held her out to
be his daughter; and
C. Crystal only has the surname Stevens because petitioner did not elect to
retake her maiden name after petitioner's divorce from Mr. Stevens.
10. Petitioner requests that Crystal's surname be formally changed to Crystal's
biological father's surname because:
