3301 (d) OF THE DIVORCE CODE
AND NOW comes the above named Plaintifl: Linda Kay Woltz. by and through her
attorneys. Mark. Weigle and Perkins and David p, Perkins. Esquire. and seeks t? obtain a Decree
in Divorce from the above-named Defendant. Jerry Richard Woltz. upon the grounds hereinafter
more fully set forth:
I. Plaintifl: Linda Kay Woltz, is a sui juris adult individual residing at 421 First
Street. Carlisle. Cumberland County. Pennsylvania since March. 1995.
2. Defendant, Jerry Richard Woltz. is a sui juris adult individual residing at 53 South
High Street. Newville. Cumberland County. Pennsylvania since February. 1995.
3, The Plaintiff and the defendant are nationals and citizens of the United States of
America. and both have been bona tide residents of the Commonwealth of
Pennsylvania. for at least six (6) months previous to the tiling of this Complaint in
Divorce.
4. The Defendant resides in the County of Cumberland. Commonwealth of
Pennsylvania. where Jerry Richard Woltz. has resided for a period in excess of six
(6) months.
5. The Plaintiff and Defendant were married on January 19. 1969. at Newville.
Cumberland County. Pennsylvania.
6, There have been no prior actions of divorce or tiJr annulment between the parties
in this or any other jurisdiction.
7, Plaintiff has been advised that counseling is available and that Plaintiff may have
the right to rcquest that the court rcquirc the parties to participate in counseling.
8, The marriage is irretrievable broken.
