COMPLAt.ln:
1. Plaintiff is a corporation having offices in Bedfford Par~, Illinois.
2. Defendant is a corporation having its offices and place of business at 110
Limekiln Road, New Cumberland, Cumberland County, Pennsylvania 17070.
3. ~'o induce the extension of credit, Defendant executed and delivered to
Plaintiff its Application for Credit, a, true and correct copy of which is attached
hereto, marked Exhibit "1", and made a part hereof.
4. On or about August 5, 1994, Plaintiff and Defendant entered into a written
Advertising Contra~t, a true and correct copy which is attached hereto, marked Exhibit
II 2 ", and made a part hereof.
5. Plaintiff avers that Defendant is in default of the Agr"\lment by having not
made payment to Plaintiff as promised, thereby rendering the entire balance immediately
due and payable.
6. Plaintiff avers that as of February, 1995, said pol.tcies were cancelled or.
otherwise terminated. A copy of cancellation letter is attached hereto, marked Ixhibit '
113 It I and made a part hereof.
7. Plaintiff avers the COlltract between the parties provides a lump sum paylilent
equal to all amounts due and which would otherwise have become due to the next
anniversary of the effectiv~ date at the full monthly unit charges.
