the consideration of the within petition and upon review of
the Custody and Guardianship Agreement attached to that
Per.ition as Exhibit "A" which has been executed by the parents
of the Child, Christine Lacole Evans, being Charles Russell
and patricia Evans, IT IS HEREBY ORDERED AND DECREED THAT:
1. Ivy K. Logue and Brian M. Logue are hereby appointed
as sole and physical custodians and guardians of Christine
Lacole Evans (hereafter "the Child") .
2. As legal and physical custodians and guardians of
the Child, Ivy K. Logue and Brian M. Logue shall have all
rights and obligations pertaining to the Child the same as if
they were her nature parents including, but not limited to the
right and obligations to:
a. Provide shelter and food.
b. Provide medical treatment.
c. Provide health insurance coverage.
d. Provide necessary education.
e. Provide day care.

Have access of all records and information
pertaining to the Child.
5. For the past five (5) years until approximately May
1, 1996 the subject Child resided with her above named pare~ts
at the legal residence.
6. The above named parents of the subject Child,
Charles Russell and patricia Evans, have agreed to appoint the
petitioners, Brian M. Logue and Ivy K. Logue, as custodians
and guardians of the subject Child, Christine Lacole Evans as
set forth and indicated on the Custody and Guardianship
Agreement attached hereto marked Exhibit "A" and made a part
hereof.
7.
The petitioners have not participated as a
party, witness or in any other capacity in any other
litigation concerning the custody or guardianship of the
subject Child in this or in any other court.
8. The Petitioners have no information pertaining to
any custody or guardianship proceedings concerning the subject
child pending in a court of this Commonwealth.
9. Your Petition~rs do not know of any person not a
party to this proceedings who has physical custody of the
Child or claims to have custody or guardianship with respect
to the Child.
10. The best interest and permanent welfare of the Child
would be served by the granting the petitioners custody and

guardianship of the Child as agreed upon by the Petitioners
and the Child's parents, Charles Russell and Patricia Evans as
