IN THE COURT OF COMMON PLEAS OF CUMBERLAND COUNTY, PENNSYLVANIA CIVIL ACTION - LAW JEREMY A. FISHEL and STEPHANIE E. DORSEY, Defendants t 96-')03, IN CUSTODY CUSTODY ITIPULATION AND AGRBBMBNT THIS AGREEMENT AND STIPULATION entered into the day and year hereinafter set forth, by and between JOANNE E. DORSEY, (hereinafter referred to as "Grandmother"), and JEREMY A. FISHEL and STEPHANIE E. DORSEY (hereinafter referred to as "Parents") . WHEREAS, the Plaintiff is the maternal grandmother of the child. WHEREAS, the Defendants are the natural parents of Jeremy A. Fishel, Jr., born March 26, 1996, (hereinafter referred to the "Child"); and WHEREAS, the parties wish to enter into an agreement relative to custody of the child; and NOW, THEREFORE, in consideration of the mutual convenants, promises and a9reem~nts as hereinafter set forth, the parties agree as follows: 1. The parties shall have shared legal and physical custody of the child. 2. The parties will keep one another advised immediately relative to any emergencies concerning the child and shall further take any necessary steps to insure that the health and well being of the child is protected. 1 3. Grandmother and Parents shall have equal access to all records and pertinent information regarding the ohild's well- being that may be generated by educational, medical, psyohological and other protes8ionals. Grandmother and Parents shall discuss major decisions relating to the child's health, education, social, emotional, spiritual, and materials needs. 4. Grandmother acknowledges that by entering into this Custody stipulation and Agreement she may be liable for support of the child. 5. Parents agree to execute the necessary Grandmother to claim child as a deduction Grandmother provides support for the child. 6. None of the parties shall do anything which may estrange the child from the other parties, or injure the opinion of the ohild as to the other parties, or which may hamper the free and natural development of the child's love and affection for the other parties. 7. Any modification or waiver of any of the provisions this Agreement shall be effective only if made in writing only if exeouted with the same formality as this stipulation Agreement. 8. The parties desire that this stipulation and Agreement be made an Order of Court to the Court of Common Pleas of Cumberland County, and further acknowledge that the Court of Common Pleas of Cumberland County does, in fact, have documents for in the event of and and 2 ,I I' " , jurisdiction over the issue of custody of the minor ohild and shall retain such jurisdiction should circumstances change and any ot the parties desires or require modification of said Order. 9. The parties agree that in making this Agreement, there has been no fraud, concealment, overreaching, coercion or other unfair dealing. 10. The parties acknowledge that they have read and understand the provisions of this Agreement. party Each acknowledges that the Agreement im fair and equitable and that it is not the result of any duress or undue influence.
