1. The pllllntiff, Marl n. .lones, Is Iln IIdull indlvidulIl residing at 5
East I~uther Street, Carlisle, Cumberland County, Pennsylvania 17013.
2. The defendant, .lames Robert Hodge, (SSN: 206-38-7861) (OOR: 8/10/46),
is an adult indlvidulIl residing at 55 East North Street, Carlisle, cumberland
County, Pennsylvania, t7013.
3. The defendant has had IIn Intimate relationship with the plaintiff.
4. Since approxlmlltely August, 1995, the defendant has attempted to
clluse and has intent !onally, knowingly, or recklessly cllused serious bodily harm,
to the plaintiff, has placed the plllintiff and her minor grandson, Baaron Rernard
Carthorn in rellsonable fear of Imminent serious bodi Iy injury, and has knowingly
engaged In II course of conduct or repeatedly committed act:; toward the plaIntiff
and her minor grnndson ami, under c I rCllmstllnces which have placed the them In
reasonahle feM of bodi Iy injury. This has included, but is not I imlted to, the
following specific instllnces of abuse:
II) On or IIbout MIlY 26, 1996, the dcfendllnt walked by the
plllint iff's residence Ilwl threlltelled her yell ing, "You haven't
gotten every thing you're gonnll get, bitch; this ain't through; I'm
gonna fuck you up Ilgllin Ilnd thnt Ii tt Ie llog (referring to the
plnlntiff's 8 year old grnndson, I.larron l1erOlml Carthorn, who lives
with her) of yours." A condition of the defendllnt's ROR hili I Is
that he not hllve IIny contllct with the pllllntlff,
h) On or IIbout Mny 20, ICJCJti, the defendllnt yelled ohscenlties lit
the plaintiff, punched her wlth his fists ahout her hencl and fnce,
pushed her, kicked her IIbout her legs IImlankles, kicked her In the
rectal area, IInd when she turned around, he kicked her In the
abdomen, A witness to this Incident yelled lit the defendllnt to stop
and he ran, The plaintiff telephoned the Carlisle police, who
IIpprehended the defendant, arrested him, charged him with simple
assault, and released on his own recognizance, The plaintiff sought
medical treatment at the Carlisle Hospital. She Bustlllned bruising
and swelling IIbout her face, soreness ahout her face, legs, and
rectal area, and a punctured Intest Ine which she was advised would
require surgery.
c) In or about early Mny, 1996, the defendllnt pushed ami shoved
the plaint I ff about, threatened to hllve his fllmlly memhers beat her
up, threatened to hllve her Il yellr old grandson kl I led or ahducted,
IInd threlltened that if she let the child outside, she would find him
dead.
tI) In or nhout ,T/lnuary, 1996, the tlefendllnt shoved the plaintiff
causing her to filii to the floor IIntl stole $ti~,OO from her.
e) On or Ilbout August 29, 199~, the defendllnt, swinging II wooden
bed s I /It like II lmscb/1I1 bll t, struck t he 1'1 II I n t iff in the f lice, then
twice Ilcross the hllck. The IIp/lrtment mlllntenllnce mlln telephoned the

simple IISSIIUlt, IInd confined to Cumherlllnd County Prison fnr
approximntely J.1 days, The plldntiff sought medical /lttentlon at
Cllrlisle IInspitlll, She sustained hruislng Ilhout her fllce IInd hllCk,
/lncl she lo,st II tooth /IS II result of this incident, During this
Incident the defend/lnt /llso c/lused signlfic/lnt dam/lge to the
plaintiff's Ilpllrtment door which resulted in the IlIndlord
withholding $150.00 from her security deposit to pay for the
damllges. See IIltllched Exh Ibi t A, incorpornted by reference.
f) Since IIpproximlltely August, 1995, the defendllnt hils IIhused the
plllintiff in wllYs including, but not limited to, PUshing, shoving,
Slapping, punChing, kicking, choking, IInd spitting on the plllintiff.
During one incident the defendant wrllpped a belt llround the
plaintiff's neck /lnd choked her. The defend/lnt h/ls threatened to
ki II the plaint! ff on severn I occ/lsions, IInd hils made slmi lar
threats to her R year old gr/lndson, A/lamn Aernard Carthorn.
5. Tht! pllllnt i ff believes IInd therefore livers that she IInd her grandson
are In Immediate and present danger of abuse from the defendant ancl that they are
in need of protect Ion from such IIbuse.
6, The plaintiff desires that the defendant bt! prohihlted from having
/Iny direct or indirect contact with the plaintiff or her minor grnndchild
