TO THE HONORABLE JUDGES OF SAID COURT;
1. The Plaintiff is Mary K. Haga residing at 1444 Apple
Circle,
Apartment
168,
Mechanicsburg,
Cumberland County,
Pennsylvania 17055.
2. The Defendant is Mark G. Haga, who is a truck driver who
uses the address of Twin x, Inc., 14324 21st Avenue North,
Plymouth, MN 55447.
3. Plaintiff seeks legal and primary physical custody of the

pro~eedings who has physical custody of the children or claims to
have custody or visitation righte with respact to the children.
7. The best interest and permanent welfare of the children
will be served by granting the relief requested because:
A. Plaintiff has participated in the primary care of
the children throughout their lives.
B. Plaintiff can provide a stable environment.
c. Plaintiff can provide a loving home.
a. Each parent whose parental rights to the children have
not been terminated and the person who has physical custody of the
children have been named as parties to this action.
WHEREFORE, Plaintiff requests this Honorable Court to grant
her legal and primary physical custody of her children subject to
specific periods of temporary physical custody to the Defendant.

1. The Mother, Mary K. Haga, and the Father, Mark G. Haga, shall
have shared legal custody of Alexander John Haga, born July 2, 1989, and
Kane Anders Haga, born November 30, 1992.
2. The Mother shall have primary physical custody of the
Children.
3. The Father shall have reasonable periods of partial physical
cuatody of the Children as arranged by mutual agreement of the parties.
The Mother agrees to cooperate in scheduling periods of partial custody to
coordinate with the Father's work schedule.
4. This Order is entered pursuant to an agreement reached by the
parties at a CUstody Conciliation Conference. In the event either party
desires to modify this order, that party may petition the Court to have the
case again scheduled with the Custody Conciliator for a Conference.

2. A Conciliation Conference was held on July 31, 1996, with the
following individuals in attendance: The Mother, Mary K. Haga, with her
sister-in-law, Terry Hardy and her legal counsel, Thonas D. Gould, Esquire.
The Father, Mark G. Haga, who is a truck driver currently residing in
MiMesota, did not attend the Conference. The Conciliator spoke with the
Father by telephone during the Conference and confirmed that the Father had
received notice of the Conference date but was unable to attend. The
Father explained his position on custody arrangements to the Conciliator by
telephone.
3. The parties agreed to entry of an order in the form as attached.
