COMPLAINT FOR CUSTODY
1. The plaintiff is Wendy J. White, 520 Third Street,
Carlisle, Cumberland County, Pennsylvania, 17013.
2. Defendant is Robert D. White, Sr., who 760 East
Louther street, Carlisle, Cumberland County, Pennsylvania,
17013.
3. Plaintiff seeks custody of the following child:
Robert D. White, Jr. born March 9, 1992
The child was born of the marriage of the parties.
The child is presently in the custody of
plaintiff.
The child has resided with both parenta as
follows:
Since October 25, 1995 with plaintiff at 520 Third
street, Carlislel
From March 1994 to October 1995 with both parents
at 760 East Louther Streetl
The parties lived at 736 East Louther Street at
time of birth of the child.
4. The relationship of plaintiff to the child is that
of mother. The plaintiff currently resides with the child.
5. The relationship of defendant to the child is that
of father. The defendant currently resides with Kim
Hedrick.
6. Plaintiff has not participated as a party or
witness, or in another capacity, in other litigation
concerning the custody of the child in this or another
court.
Plaintiff has no information of a custody
proceeding concerning the child pending in a court of this
Commonwealth.
Plaintiff does not know of a person not a party to
the proceedings who has physical cuatody of the child or
claims to have custody or visitation rights with respect to
the child.
7. The best interest and permanent welfare of the
child will be served by granting the relief requested
because the child should have a close and continUing
relationship with both parents and plaintiff has been the
primary caretaker of the child.
8. Each parent whose parental rights to the child have
not terminated and the person who has physical custody of
the child have been named as parties to this action.
WHEREFORE, plaintiff requests the court to grant
primary custody of the child to her.

AND NOW, this ~ day of , 1996, upon
consideration of the attached Custody Co ci1iat on Report, it is
ordered and directed as followsl
1. The Mother, Wendy J. White, and the Father, Robert D. White,
Sr., shall enjoy shared legal custody of Robert D. White, Jr.,
born March 9, 1992.
2. The Mother shall enjoy primary physical custody of the minor
child.
3. The Father shall enjoy temporary physical custody of the minor
child as follows:
A. On alternating weekends from when Father gets out of work
on Friday afternoon until Monday morning at 7:30 a.m.
B. On every Wednesday from when the Father gets out of work
until Thursday morning at 7:30 a.m.
C. On the weekdays when Father is not working or when he
gets out of work and the Mothor is still at work, Father
may pick the child up at the day care and have custody
until 5100 p.m., when he shall return the child to the
Mother's custody.
D. At such other times as agreed upon by the parties.
4. Both parents shall enjoy two weeks of vacation with the minor
child which shall be consecutive. The parents shall notify
each other prior to May 30th of each year as to when they
intend to exercise the two-week vacation. In the event the
proposed vacation conflictD between the parents, the parent
who notifies the other parent first in writing shall have
preference.
