2, The mnrriage of Plninllff and Defendant is irretrievably broken and ninety dnys hnve
elapsed from lhe dale of filing and service of the Complaint.
3, I consent to the entry of nlinul decree of divorce withoul no lice,
4. I understnnd thntl may lose righls concerning nlimony. division of property. Inwyer's
fees. or expenses if i do not claim them before a divorce is grnnted,
5, I understand that I will nol be divorced until a divorce decree is enlered by the Court and
that a copy of the decree will be sent to me immediately after it Is filed with the Prolhonotnry,
6, I have been advised of the availability of murrlage counseling and understand thnt I mny
request thai the court require that my sponse and I pW'licipate in counseling,
