PETITION FOR LETTER ROGATORX
The petition of Janice B. Andrews and Allstate Insurance
Company, defendants in the above numbered and entitled cuase,
with respect, represents:
1.
Sandra Mulkey has sued Janice B, Andrews and Allstate
Inaurance Company for injury sustained because of an automobile
accident which occ~rred in Jef.ferson Parish, Louisiana on or
about September 23, 1994.
2.
That petitioner, Allstate Insurance Company, has noticed the
records deposition of Gallagher Bassett Services to obtain copies
of a claim file pertaining to claims for insurance payments made
I by Sandra Mulkey arising out of an accident which occurred on
June 29, 1995.
WHEREFORE, petitioner prays that letter rogatory issue to
the appropriate authority in Carlisle, Pennsylvania commissioning
Allstate Insurance Company to take the records deposition of
Basaett Services,
