1. Plaintiff is Rebecca D. Reed, residing at 13308 US 24, Cecil, Ohio 45821.
2. The defendant is Patrick M. Blaine, residing at 16 Annendale Drive,
Carlisle, Pennsylvania 17015.
3. Plaintiff seeks custody of Jonathan Philip Blaine, born March 20, 1990,
and Zachary Michael Blaine, born AprilS, 1993.
4. The children were not born out of wedlock. The children are presently in
the custody of Rebecca D. Reed who has had custody ofthe children since August, 2004.
5. For the past five years, the child has resided with the following persons at
the following addresses:

6. The mother ofthe children is Rebecca D. Reed who currently resides at
13308 US 24, Cecil, Ohio. She is married.
7. The father of the children is Patrick M. Blaine currently residing at 15
Annendale Drive, Carlisle, Pennsylvania. He is single.
8. The plaintiff currently resides with her husband, and her two children,
Jonathan Philip Blaine and Zachary Michael Blaine.
9. The defendant currently resides alone.
10. Plaintiff has not participated as a party or witness, or in another capacity,
in other litigation concerning the custody of the children in this or another court.
11. The plaintiff has no information of a custody proceeding concerning the
children pending in a court in this Commonwealth or in any other court.
12. Plaintiff does not know of a person not a party to the proceedings who has
physical custody of the children or claims to have custody or visitation rights with respect
to the children.

13. The best interest and permanent welfare of the children will be served
because plaintiff is better able to provide for the needs of the children.
14. Each parent whose parental rights to the children have not been terminated
and the person who has physical custody of the child have been named as parties to this
action.
WHEREFORE, plaintiff requests this Court to award herprimary physical custody of
Jonathan Philip Blaine and Zachary Michael Blaine.

it is hereby directed that parties and their respective counsel appear before John J. Mangan, Jr., Esq.
at 4th Floor, Cumberland County Courthouse, Carlis~ on Friday, February 23, 2007
, the conciliator,
at 1:00 PM
for a Pre-Hearing Custody Conference. At such conference, an effort will be made to resolve the issues in dispute; or
if this cannot be accomplished, to define and narrow the issues to be heard by the court, and to enter into a temporary
order. All children age five or older may also be present at the conference. Failure to appear at the conference may
provide grounds for entry of a temporary or permanent order.
The court hereby directs tbe parties to furnish any and all existing Protection from Abuse orders,
Special Relief orders, and Custody orders to the conciliator 48 hours prior to scheduled hearin2.
