5, At the time of the accident, Edward C. Long, Jr. wes Insured under a
policy Issued by the AlIstete Insuranoe Company, Policy Number 028911660, with bodily
Injury liability limits of $25,000 per person/$50,OOO per occurrence. A true and correct copy
of the Certlflcatlon of Polley Limits Is attached hereto and Incorporated herein by referenoe
as EKhlblt "C".
6, The decedent, David J. Twletmeyer, was born on December 19,1975
and was 20 years old at the time of the accident. The decedent was never married and
Is not survived by any children,
7. The only surviving heirs or the decedent are his parents, Ronald J,
Twletmeyer and Chong Kim TWletmeyer,
8, SUbsequent to the accldent, the Allstate Insuranoe Company
approached Mr, Twletmeyer to resolve all first party and liability claims ariSing out of the
accident. Allstate has agreed to pay the polley limits of $25,000 to settle all wrongful death
and survival action claims arising out of the accident. On behalf of himself, his wife and
the Estate, Ronald J, TWletmeyer has agreed to accept the settlement.
9. Allstate has tendered and Ronald J, Twletmeyer has signed the
Release which Is attached hereto and Incorporated herein by reference EKhlblt "D".
