The allegations set forth in parsgraphe 1 through 6 above are incorporated herein by roference. 8. Ae a direct and proximate reeult of the negligent, careless and/or rec~less acts of the Defandant, Blizabeth aebert, the Plaintiff, Robert Chasteen, has suffered injuriee which were and are severe, painful, serious and permanent. Theee injuriee include, but are not limited tOI (8) a severely herniated disc of his spine (t..., LIS 81) which required surgical removal and, subsequently, a spinal fusion, also necessi tating the removal of bone from pelVis/hip area to do fusion (b) nerve damage, and (0) numbnees and tingling. g.
