1. The Plaintiff is Rafael B. Bellido residing at 879 Mandy
Lane, Camp Hill, Cumberland County, Pennsylvania 17011.
2. The Defendant is Lisa M. Forrester, residing at 905
Hlunmel Avenue, Lemoyne, Cumberland County, Pennsylvania 17043.
3. Plaintiff seeks oustody of the following child.

CUSTODY AGRIIMINT
THIS AGREEMENT, made this ~ day of~, 1996, by
and between Rafael B. Bellido, hereinafter referred to as Father,
and Lisa M. Forrester, hereinafter referred to as Mother.
WITNESSETH I
WHEREAS, Mother and Father are the parents of Consuelo L.
Bellido, born July 7, 1995, and
WHEREAS, Mother and Father wish to enter into this Custody
Agreement to settle and resolve the outstanding issues regarding
the custody of their child,
NOW THEREFORE, it is agreed by the parties thatl
1. Father shall have legal custody of the child. Mother and
Father are to have equal access to the child's personal I school and
medical records and each party is to inform the other of
signifioant events that occur in the child's life,
2. Father shall have primary physical custody subject to
Mother's periods of visitation and/or partial custody at times
mutually agreed upon by the parties.
