PA 17019-0310 RE: Christina Miller HMC# 694601 Dear Mr, Schrack: Christina Miller is a patient of mine who has been treated for an L2 flexion distraction injury, She sustaincd the injury on September 17, 1995, as a restrained passenger involved in a motor vehicle accident. She was initially taken to Polyclinic Hospital and subsequently transferred to Hershey Medical Center, There was ecchymosis noted over the abdominal region; she complained of some back pain. Radiographs revealed a flexion distraction injury of the 1.2 vertebrae, Her molor examinalion was enlirely normal, 515 molor strength for all muscle groups of the upper anu lower extremities, There was a question of having decreased sensation to light touch and pin prick over the right and left anlerior medial thigh uermatomes, She was placed into a hyperextension body cast to allow for possible osseous healing of the spinal fracture, Good alignment of lhe vertebral column was achieved with lhe hyperextension castalld she was discharged to home on September 21. 1995, She was seen in Pediatric Orthopaedic Clinic on October 7, 1995, She had been tolertlling her cast quite well. She was seen again on November 21, 1995. for follow.up and x-rays revealed no uefinite evidence for healing. She was taken out of her cast and placed into un overlap bruce, She hud 18 degrees of kyphosis meusured from Ll to L3. She had pain to palpution over lhe kyphotic area of the lumbar spine, She noted a pulling sensalion in lhe area of her back during physicul therupy, At this time, I felt thaI shc was a candidule for openHi ve intervention, There was no definite evidence of healing on rudiographs to suggest that with continued conservulive treatment her spine injury would heal. She also had progressive kyphosis noteu radiographically EXHIBIT "B" ,\11 blll.ll 01'IIU11111111> l'rH\...r'll~ Prepared for: Claim number: Proposal number: JESSICA MILLER 38-7115-081 04 Requested by: Office name: Location code: Date prepared: CWOLF HARRISBU 114 12-06-96 PROPOSAL NOT VALID AFTER 01-31-97 Schedule of Annuity Payments Total Cost Payments Seqrnent 1 A SINGLE PAYMENT OF $11,476.72 $11,476.72 $6,000.00 ON APRIL 3, 2008.
