l\ND tOl, canes the plaintiff, Tanya Moore, by and through her attorneys,
O'BRIEN, BARIC & SOiERER, and respectfully represents as follows:
1. The plaintiff is Tanya Moore, an adult individual residing at
88 Frytown Road, Newville, Currberland County, pennsylvania.
2. The defendant is Jeremy S. Belt, an adult individual residing
at 1282 Boiling Springs Road, Boiling Springs, Currberland County,
Pennsylvania.
3. plaintiff seeks custody of the following child:

The child was born out of wedlock.
The child is presently in the custody of plaintiff, who resides at
88 Frytown Rood, Newville, Currberland County, Pennsylvania.
During the past five years, the child hae resided with the

The natural father of the child is Jeremy S. Belt, currently
residing at 1282 Boiling Springs Road, Boiling Springs, Pennsylvania.
He is not married.
4. The relationship of the Plaintiff to the child is that of
natural !rother. The plaintiff currently resides with the following persone:

5. The relationship of the Defendant to the child is that of
natural father. The defendant currently resides with the following persons:

6. Plaintiff has not participeted as a party or witness, or in
any other capacity in other litigation, concerning the cuetody of the child in
this or in any other Court.
Plaintiff hae no information of a custody proceeding concerning
the child pending in a court of this Ccmronwealth.
Plaintiff does not know of a person not a party to the proceedings
who hae physical custody of the child or claims to have custody or visitation
rights with reepect to the child.
7. The best interest and peIl1'<Ulent welfare of the child will be
eerved by granting the relief requested because the plaintiff, Tanya Moore, is
best able to provide primary physical custody of the child.

8. Each parent whose parental rights to the child have not been
terminated and the person who hae physical cuetody of the child have been
named as partiee to this action. All other persons, named below, who are
known to have or claim a right to custody or vieitation of the child will be
given notice of the pendency of this action and the right to intervene. tOlE.
