PETITION FOR CHANGE OF NAME
AND NOW comes Petitioner, Angela L. Richcreek, by and
through her attorney, Cindy L. Koser, Esquire, and in support of
her Petition For Change of Name of her minor daughter, Brandie
Nichole Sprigle, respectfully represents as follows:
1. Petitioner, Angela L. Richcreek, is an adult individual
who currently resides at 221 S. Washington St., Mechanicsburg,
Cumberland County, PA 17055.
2. Petitioner is the natural mother of Brandie Nichole
Sprigle whose name change is the subject of this Petition.
3. Brandie Nichole Sprigle is a minor child who was born
on September 6, 1983 in Fayetteville, North Carolina.
4. At the time of the child's birth, the Petitioner was
married to Donald Charles Sprigle, whose name appears on the
child's birth certificate (copy attached as Exhibit A).
5. Donald Charles Sprigle is not the natural father of the
Brandie Nichole Sprigle.
6. The Judgment in Divorce (attached hereto as Exhibit B)
granted to Angela L. Sprigle (now Angela L. Richcreek) and Donald
Charles Spriyle made a factual finding that there were no
children born of the marriage.
7. The natural father of Brandie Nichole Sprigle was not
determined.
8. Petitioner was married to James Richcreek on September
11. 1995, and they presently live together as husband and wife.
