AND NOW, comes the claimant, TOWNSHIP OF HAMPDEN, by the President of its Board
of Commissioners, who files the following claim against .Tohn M. Blessing, .Tr. all that
certain tract of land including improvements thereon, owned by them and described
as follows:

For sewer services rendered from 11/30/01 to 10/01/02. ~aid services were rendered
pursuant to ordinance. Attached hereto is a statement of the kind and character of the
services so rendered and the price charged therefor, said statement being a true copy of
the owner's account with Hampden Township which sum of $426.62 was duly assessed
against the hereinabove-named owner and the premises on 10/01/02 and for which sum, with
interest, penalties and costs, a lien is claimed against the above-described premises in
accordance with the Act of May 16, 1923, P.L 207 (53 P.5. 5action 7101, et seq.), as
amended and supplemented, and the Act of May Z, 1945, P.L. 382 (53 P.5.5action 301,
et seq), as amended and supplemented.
