I, JACQUELINE A. KELLY, being duly sworn according to law, depose and state that
service of a copy of the Statement of Rights Upon Appointment of a Guardian, a copy of which
is attached, was made on EDNA S. CHASE, by reading a copy of it to her on January 17, 2007,
at 4:00 p.m. at The Gardens of the Bridges at Bent Creek, 2100 Bent Creek Boulevard,
Mechanicsburg, Pennsylvania, 17050. I read the Statement of Rights to the alleged incapacitated
person to the maximum extent possible, in language and terms she was likely to understand.
