Plaintiff is Chambersburg Hospital located at 112 North 7th Street, Chambersburg, PA 17201. 2. Defendant is James pague who resides at 17 Wyrick Avenue, Shippensburg, PA 17257. 3. From between May 16, 1998 and May 21, 1998 Chambersburg Hospital provided medical services to James pague at patient No. 210393- 5. The unpaid balance owing to Chambersburg Hospital for those medical services is $4,063.50. 4. On or around July 22, 1998 Chambersburg Hospital provided medical services to James pague at patient No. 825397-3. The unpaid balance owing to Chambersburg hospital for those medical services is $3,567.00. 5. Chambersburg Hospital has requested that James Pague pay the aforesiilid balances set forth above, however, James pague has not paid the same. 6.
