PETITION FOR NAME CHANGE
1. The Petitioner in this matter is Samuel Lee Bowers who is an adult individual
who currently resides at 25 Stone House Road, Carlisle, Cumberland County,
Pennsylvania 17013.
2. The Petitioner has resided at his current residence, 25 Stone House Road,
Carlisle, Pennsylvania, throughout the five-year period immediately prior to the filing of
the Petition for Name Change.
3. The Petitioner wishes to change his name formally to Sam Jacobs Bowers. "1
wish to do this because my original family name was Jacobs, and I wish to incorporate
the family name as part of my own name."
· If this Petition is approved, the Petitioner intends to use the name Sam
Jacobs Bowers consistently, non-fraudulently, and exclusively.
· It is not the purpose of this Petition to avoid any taxes or any other debts.
