consideration of the attached Custody Conciliation
and directed as follows:

Report, it is ordered
1. The Father, Matthew W. Monismith, and the Mother, Jennifer J.
Repa, shall have shared legal custody of Aliyah Laura Monismith, bom April
26, 1996. Each parent shall have an equal right, to be exercised jointly
with the other parent, to make all major non-emergency decisions affecting
the Child's general well-being including, but not limited to, all decisions
regarding her health, education and religion.
2. The parties shall share having physical custody of the Child on a
weekly basis with the exchange of custody to take place each week on Sunday
at 7:00 p.m.
3. The parties shall share or altemate having custody of the Child
on holidays as arranged by agreement of the parties.
4. The Mother shall ensure that the Child is not left alone in the
care of the Mother's boyfriend, Timothy White.
5. This Order is entered pursuant to an agreement of the parties at a
Custody Conciliation Conference. The parties may modify the provisions of
this Order by llRltual consent. In the absence of llRltual consent, the terms
of this Order shall control.
