for sewer services rendered from July 1, 1996 through February 29, 2000. Said
services were rendered pursuant to applicable rate resolutions and ordinances.
Attached hereto is a statement of the kind and character of the services so
rendered and the prices charged therefor, said statement being a true and correct
copy ofthe Billing Worksheet for Owner's account with South Middleton Township
Municipal Authority, which sum of $1,268.58 for the above period was duly assessed
against the above Owner and premises beginning July 1, 1996 and for which sum,
