for sewer assessment against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is Carlisle Suburban Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name of the owners of the property against which this claim is filed are Penta Star
Investments, Inc. with an address at the following:
475 Sherwood Drive
Carlisle, P A 17013

A description of the property against which this lien is filed is attached hereto and marked
Exhibit "A".

This lien is claimed for assessment issued against the above described property for installation of
public sewer lines in the amount of $43,875.00.

Interest is claimed on the above-mentioned amount from November 15, 1999 and costs of filing
are also claimed.

Commonwealth of pennsylvania, by its solicitor, Broujos & Gilroy
PC, at the request of Penta Star Investments, Inc., the Defendant
herein, and for and in consideration of the sum of One Dollar
($1.00) to it in hand paid by the Defendant at the time of the
execution hereof, the receipt whereof is hereby acknowledged,
does for itself, its successors and its assigns, release from the
lien of said judgment the real estate known and numbered as Lot
Number 70R, Phase A Village of Cross Creek, copy of the legal
description of which is attached hereto and made a part hereof.
