MECHANIC'S LIEN CLAIM
1. Claimant, Mechanicsburg Carpet Mart, Inc. (Carpet Mart), is a
Pennsylvania corporation with a principal place of business at 5103 Carlisle Pike,
Mechanicsburg, Pennsylvania, 17055. Carpet Mart files this Mechanic's Lien Clairn as
contractor.
2. The owners of the property subject to the Lien are Defendants John T.
Lewis and Claudia R. Girrbach (Defendants) with a principal residence at 301 Harmony
Lane, Carnp Hill, Pennsylvania, 17011.
3. The last date on which Carpet Mart performed work for which this Claim is
made was Novernber 21,1999.
4. This Mechanic's Lien Claim arises from a contract between Carpet Mart
and the Defendants wherein Carpet Mart agreed to furnish all flooring and related
materials required for the alteration and repair of the Defendants' home located at 301
Harmony Lane, Camp Hill, Pennsylvania. A copy of Carpet Mart's invoice dated

November 17, 1999, evidencing its contract with the Defendants, is attached as Exhibit A
and incorporated by reference.
5. The amount claimed to be due is $12,458.66, plus interest in the amount of
1.5% per month from December 21,1999, forward.
6. The improvements and the property claimed to be subject to this Lien is the
property owned by John T. Lewis and Claudia R. Girrbach and all ofthe irnprovernents
thereon located at 301 Harmony Lane, Camp Hill, Cumberland County, Pennsylvania,
17011, tax map # 10-20-1846-181.

AFFIDAVIT OF SERVICE OF NOTICE
Noreen A. Mareski, an adult, being duly sworn according to law, deposes and says
as follows:
1. I am an adult and a Paralegal with Hartman Underhill & Brubaker LLP,
counsel for Clairnant in the above-captioned proceedings, and submit this
Affidavit pursuant to 49 P.S. ~ 1502 (a) (2).
2. At 4:00 P.M. on March 15, 2000, pursuant to the authority granted me by
the claimant, Mechanicsburg Carpet Mart, Inc., I served upon John Lewis, Notice
of the filing of the Mechanic's Lien Claim filed herein, bearing the Court term and
number and date of filing of the said claim, by personally handing the said Notice
to John Lewis at his home, 301 Harmony Lane, Camp Hill, Pennsylvania 17011.
A true and correct copy of the Notice of Claim is attached hereto, made part hereof
as Exhibit A.
