The Petitioner, Amy L. Benoit, by and through her attorney, Keith B. DeArmond,
Esquire, requests that this Court order a change of name for her, pursuant to 54 Pa. Cons.
Stat. Section 701 et seq., and in support thereof represents as follows:
1. Petitioner was born on August 30, 1983
2. Petitioner, Amy L. Benoit, is a resident of812 Kent Drive, Mechanicsburg,
Cumberland County, Pennsylvania, and has resided at the following locations during the
five years next preceding the fi1ing of this petition: 812 Kent Drive, Mechanicsburg,
Pennsylvania, from 8/99 to present; 2 Mayfair court, Camp hill, Pennsylvania, from 11/97
to 8/99; 5029 East Trindle Road #7, Mechanicsburg, Pennsylvania, from 6/93 to 11/97.

There are no judgments or decrees of like character or record against the

Lee Savarese.
5. The reasons for the requested change of name are the following:
(a) To conform Petitioner's name with the name of her natural mother,
with whom Petitioner presently resides and has continuously resided
since her birth.
(b) Petitioner is generally known throughout the community by the name
of Amy L. Savarese.
(c) Petitioner has had no contact with her natural father, whose surname
is Benoit, since 9/17/97. Prior to that dat petitioner had no contact
with the natural father since 8/15/89

PETITION FOR CHANGE OF NAME
The Petitioner, Amy L. Benoit, by and through her attorney, Keith B. DeArmond,
Esquire, requests that this Court order a change of name for her, pursuant to 54 Pa. Cons.
Stat. Section 701 et seq., and in support thereof represents as follows:
1. Petitioner was born on August 30, 1983
2. Petitioner, Amy L. Benoit, is a resident of 812 Kent Drive, Mechanicsburg,
Cumberland County, Pennsylvania, and has resided at the following locations during the
five years next preceding the filing of this petition: 812 Kent Drive, Mechanicsburg,
Pennsylvania, from 8/99 to present; 2 Mayfair court, Camp hill, Pennsylvania, from 11/97
to 8/99; 5029 East Trindle Road #7, Mechanicsburg, Pennsylvania, from 6/93 to 11/97.

5. The reasons for the requested change of name are the following:
(a) To conform Petitioner's name with the name of her natural mother,
with whom Petitioner presently resides and has continuously resided
since her birth.
(b) Petitioner is general1y known throughout the conununity by the name
of Amy L. Savarese.
(c) Petitioner has had no contact with her natural father, whose surname
is Benoit, since 9/17/97. Prior to that dat petitioner had no contact
with the natural father since 8/15/89

AND NOW, comes Petitioner, Amy L. Benoit, by and through her attorney, Susan M.
Kadel, Esquire and requests the following:
1. Movant is Amy L. Benoit an individual who currently resides at 812 Kent
Drive, Mechanicsburg, Cumberland County, Pennsylvania 17055.
2. Movant has obtained new counsel in this matter.
3. A hearing on this matter is scheduled for July 3, 2000 at 8:45 a.m., in Court Room
No.2.
4. Upon review of the file, new counsel for Movant has determined that publication
of the requested name change has not been effectuated, nor has service on the natural father been
obtained.
