STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is Carlisle Suburban Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

This lien is claimed for assessment issued against the above described property for installation of
public sewer lines in the amount of$2,61O.00.

Interest is claimed on the above-mentioned amount from November 15,1999 and costs of filing
are also claimed.

Registered Surveyor, dated September 19, 1978 and bearing drawing
No. 78l43, as follows, to wit: .
BEGINNING at a stake on the Northeast side of Channel Drive at a
corner of property now or formerly of James L. & Mary L. Lebo I
thence extending from said beginning point and along Channel Drive
North 34 degrees 30 minutes West, a distance of 79.00 feet to a
stake set at a corner of property now or formerly of Carlisle
Area School District; thence extending along same North 66 degre~s
31 minutes East, .a distance of 338,02 feet to an existing iron pin
at a corner of property now or formerly of Carlisle Area School
Districtl thence extending along same South 22 degrees 11 minutes
East a distance of 79.00 feet to a stake set at a corner of property
now or formerly of James L. & Mary L. Lebo, aforesaid; thence ex-
tending along same South 66 degrees 46 minutes 22 seconds West,
a distance of 321.13 feet to' the first mentioned stake and place
of beginning.
