Carlisle Suburban Authority, North Middleton Township, County of Cumberland,
Commonwealth of Pennsylvania, by its solicitor, Broujos & Gilroy, P.C., hereby files its claim
for sewer assessment against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is Carlisle Suburban Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name of the owners of the property against which this claim is filed are Dennis E. Hair and
Alice D. Hair with an address at the following:
29 Bellaire Avenue
Carlisle, PA 17013

A description of the property against which this lien is filed is attached hereto and marked
Exhibit "A".

The authorities under and by virtue of which this lien is filed is the Municipal Claims and Liens
Act, 53 P.S. ~7101 et seq.

This lien is claimed for assessment issued against the above described property for installation of
public sewer lines in the amount of $2,440.00.

Interest is claimed on the above-mentioned amount from November 15,1999 and costs of filing
are also claimed.

Avellue 33 fcet wide which said point is 181.54 feet measured South-
wardly from the Southcrn line of ~Iape 1 Avenue 33 feet wide and which
said point is at thu Southern line of Lot No. 48 on the hereinafter
mentioned plan of lots; thence from said beginning point by a curve
to the left having a radius of 115 fect the arc distance of 76.0 feet;
