Carlisle Suburban Authority, North Middleton Township, County of Cumberland,
Commonwealth of Pennsylvania, by its solicitor, Broujos & Gilroy, P.C., hereby files its claim
for sewer assessment against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is Carlisle Suburban Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name of the owners of the property against which this claim is filed are Michael H. Sheets
and Diane 1. Sheets with an address at the following:
44 Bellaire Avenue
Carlisle, P A 17013

A description of the property against which this lien is filed is attached hereto and marked
Exhibit "A".

The authorities under and by virtue of which this lien is filed is the Municipal Claims and Liens
Act, 53 P.S. ~7101 et seq.

This lien is claimed for assessment issued against the above described property for installation of
public sewer lines in the amount of $5,000.00.

Interest is claimed on the above-mentioned amount from November 15,1999 and costs of filing
are also claimed.

Cumberland County, Pennsylvania, bounded and described as follows:
BEGINNING at a point on the eastern line of Bellaire Avenue. which
point is situated South 22 degrees 30 minutes East a distance of
90.00 feet from the southern line of Maple Avenue (both 33 feet wide).
and at the dividing line between Lots 42 and 41 as shown on the
hereinaf.ter mentioned Plan; thence by the southern line of Lot No. 42
North 67 degree~ 30 minutes East 162.00 feet to a point; thence by
the western line of Lot No. 40 South ~o degrees 10 minutes East
125.32 fee~ ~~wthe northern line of Bellaire Avenue; thence.by the
northerq.!JP,e of Bellaire Avenue the following three courses and
dis.tance$ '.::u2-liouth 79 degrees 50 minutes West 72.24 feet to a point;
