It is hereby stipubded that:
1. Defeudant is a11 adult individual who currently resides at 76 Mifflin Ave, New Cumberland, Pa 17070.
2. Plaintiff is an adult curreutly residing at 76 Mifflin Ave, New Cumberland, Pa. 17070.
3. The defeudant is the legal parent of Gregory Lavon Haggard Jr., 173-78-7332, born on 06/27/98. The
defendant whishes to give up her legal right of custody of the aforementioned minor. The child's
biological father Gregory Lavon Haggard, is currently incarcerated in Pulaski County Prison, Missouri,
and has been there since November 28, 1998 and is expected to be their for life.
4. The plaintiff, the aforementioned minor's Grandmother, will have complete and sole custody of in all
matters concerning the child well being.
