AND NOW, comes the claimant, TOWNSHIP OF HAMPDEN, by the President of
its Board of Commissioners, who files the following claim against, Herbert and Robin
Gilsdorf, al/ that certain tract of land including improvements thereon, owned by him and
described as follows:

For sewer services rendered from 10/01/99 to 04/01/00. Said services were rendered
pursuant to ordinance. Attached hereto is a statement of the kind and character of the
services so rendered and the price charged therefor, said statement being a true copy of
the owner's account with Hampden Township which sum of $462.15 was duly assessed
against the hereinabove-named owner and the premises on 04/01/00 and for which sum,
with interest, penalties and costs, a lien is claimed against the above-described premises in
accordance with the Act of October 29, 1981, P.L. 319, No. 113, as amended.
