PETITION FOR NAME CHANGE
AND NOW comes Petitioner, Ariel Rose Kerry, by and through her custodial parent, Karen
A. Gnazzo and her attorneys Mancke Wagner Hershey & Tully, who make the following averments
in support of this Petition for Name Change pursuant to 54 Pa.C.S.. F02 pertaining to judicial
change of name for minor child.
1. Ariel Rose Kerry, a minor child, with a date of birth of September 4, 1992, is the
child of Karen A. Gnazzo and Alexander Kerry. See Birth Certificate attached
hereto, marked as Exhibit "A" and incorporated herein by reference.
2. Ariel Rose resides with Karen A. Gnazzo at 4 Briarwood Court, Camp Hill,
Pennsylvania 17011.
3. The parents were married in September, 1989 and were divorced in 1996.
4. Ariel Rose's Social Security Number is 170-74-6524. See Social Security Card
attached hereto, marked as Exhibit "B" and incorporated herein by reference,
5. Alexander Kerry presently has visitation one time per week and is paying child
support for Ariel Rose.
6. Ariel Rose has not resided with her father since her parents separated in 1994.
7. Ariel Rose is currently enrolled in the Sporting Hill Elementary School and has been
using the last name Gnazzo in class and for class-related activities.

8. Official records at Sporting Hill Elementary School have Ariel's last name listed as
Kerry in accordance with her birth certificate.
9. In the Fall of 1999, Ariel, at her request and the request of her mother, began using
Gnazzo in the classroom.
10. Authorities at Sporting Hill Elementary have allowed the use of the name Gnazzo
out of courtesy to Petitioner, however, have indicated that they require an Order of
Court so that report cards, yearbook photos, and all official records are
synchronized with the school's official records.
11. There have been several instances where insurance bills for medical appointments
have been forwarded to the wrong insurance carrier because official records
indicate Ariel Kerry.
12. By way of further explanation, some insurance carriers have billed Alexander
Kerry's policy when the bills should have been billed to Karen Gnazzo's medical
insurance policy causing bills to return as unpaid and causing additional hardship
for Karen Gnazzo.
13. The legal standard for a Petition for Name Change is best interest of the child.
14. Ariel Rose has indicated a personal preference to change her last name to Kerry-
Gnazzo.

15. School officials, while remaining neutral relative to this request, have indicated the
need for continuity between the name of record and the name used in the
classroom and all related documents, yearbook photos, and report cards.
Therefore, it is further in Ariel Rose's best interest for the court to grant the relief
requested.
16. Alexander Kerry has advised Karen Gnazzo that he would not object to the use of
a hyphenated last name for his daughter, Ariel Rose.
17. This request is in furtherance of the best interest of the child for some or all of the
following reasons:
a. There would be no disruption between the natural bonds between father and
child as visitation is on-going,
b. The social stigma or respect of a particular name in the community is neutral
and, therefore, not an issue.
c. Ariel Rose, intellectually and rationally, understands the significance of
changing her name.
d. Since there is continued contact with the natural father and he has not
expressed an objection to the hyphenated last name, Petitioner believes it
would not lead to estrangement of the father.
e. There is no fraudulent purpose behind this request for a name change.
18, Petitioner has been fingerprinted in accordance with the judicial change of name
requirements at the Cumberland County Sheriff's Office and her fingerprint card is
attached as Exhibit "C".

PETITION FOR NAME CHANGE
AND NOW comes Petitioner, Ariel Rose Kerry, by and through her custodial parent, Karen
A. Gnazzo and her attorneys Mancke Wagner Hershey & Tully, who make the following averments
in support of this Petition for Name Change pursuant to 54 Pa.C.S.. ~702 pertaining to judicial
change of name for minor child.
1. Ariel Rose Kerry, a minor child, with a date of birth of September 4, 1992, is the
child of Karen A. Gnazzo and Alexander Kerry. See Birth Certificate attached
hereto, marked as Exhibit "A" and incorporated herein by reference.
2. Ariel Rose resides with Karen A. Gnazzo at 4 Briarwood Court, Camp Hill,
Pennsylvania 17011.
3. The parents were married in September, 1989 and were divorced in 1996.
4, Ariel Rose's Social Security Number is 170,74-6524. See Social Security Card
attached hereto, marked as Exhibit "B" and incorporated herein by reference.
5. Alexander Kerry presently has visitation one time per week and is paying child
support for Ariel Rose.
6. Ariel Rose has not resided with her father since her parents separated in 1994.
7. Ariel Rose is currently enrolled in the Sporting Hill Elementary School and has been
using the last name Gnazzo in class and for class-related activities.

8, Official records at Sporting Hill Elementary School have Ariel's last name listed as
Kerry in accordance with her birth certificate.
9. In the Fall of 1999, Ariel, at her request and the request of her mother, began using
Gnazzo in the classroom.
10. Authorities at Sporting Hill Elementary have allowed the use of the name Gnazzo
out of courtesy to Petitioner, however, have indicated that they require an Order of
Court so that report cards, yearbook photos, and all official records are
synchronized with the school's official records.
11. There have been several instances where insurance bills for medical appointments
have been forwarded to the wrong insurance carrier because official records
indicate Ariel Kerry.
12. By way of further explanation, some insurance carriers have billed Alexander
Kerry's policy when the bills should have been billed to Karen Gnazzo's medical
insurance policy causing bills to retum as unpaid and causing additional hardship
for Karen Gnazzo.
13. The legal standard for a Petition for Name Change is best interest of the child.
14. Ariel Rose has indicated a personal preference to change her last name to Kerry-
Gnazzo.

15. School officials, while remaining neutral relative to this request, have indicated the
need for continuity between the name of record and the name used in the
classroom and all related documents, yearbook photos, and report cards.
Therefore, it is further in Ariel Rose's best interest for the court to grant the relief
requested.
16. Alexander Kerry has advised Karen Gnazzo that he would not object to the use of
a hyphenated last name for his daughter, Ariel Rose.
17. This request is in furtherance of the best interest of the child for some or all of the
following reasons:
a. There would be no disruption between the natural bonds between father and
child as visitation is on'going.
b. The social stigma or respect of a particular name in the community is neutral
and, therefore, not an issue.
c. Ariel Rose, intellectually and rationally, understands the significance of
changing her name.
d. Since there is continued contact with the natural father and he has not
expressed an objection to the hyphenated last name, Petitioner believes it
would not lead to estrangement of the father.
e. There is no fraudulent purpose behind this request for a name change.
18. Petitioner has been fingerprinted in accordance with the judicial change of name
requirements at the Cumberland County Sheriff's Office and her fingerprint card is
attached as Exhibit "C".

WHEREFORE, Petitioner prays that a hearing be scheduled to address the issues raised
herein.
