Carlisle Suburban Authority, North Middleton Township, County of Cumberland,
Commonwealth of Pennsylvania, by its solicitor, Broujos & Gilroy, P.C., hereby files its claim
for sewer assessment against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is Carlisle Suburban Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name of the owners of the property against which this claim is filed are Harry E. Failor, Jr.
and Helen 1. Failor with an address at the following:
6 Green Meadow Drive
Carlisle, P A 17013

A description of the property against which this lien is filed is attached hereto and marked
Exhibit "A".

The authorities under and by virtue of which this lien is filed is the Municipal Claims and Liens
Act, 53 P.S. ~7101 et seq.

This lien is claimed for assessment issued against the above described property for installation of
public sewer lines in the amount of $2,500.00.

Interest is claimed on the above-mentioned amount from November 15,1999 and costs of filing
are also claimed.

known as Green Meadow Drive, thence by the North side of the said
public street, South 61 degrees 30 minutes West a distance of 150
feet to an iron pinl thence by lands formerly of Melvin B. Landis,
now or formerly of Richard L. Landis, et ux, North 28 degrees 30
minutes West a distance of 150 feet to an iron pinl thence by land
now or formerly of J. Mac Stine, et ux., North 61 degrees 30
minutes East a distance. of 150 feet to an iron pinl thence along
lands now or formerly of Jackson M. Smith, et ux., and Harold
Bottgenbach, South 28 degrees 30 minutes East a distance of 150
feet to an iron pin on the North side of the above mentioned public
street.
CONTAINING 22,50,0 square feet, more or less. Having thereon
erected a dwelling house known as and numbered 6 Green Meadow
Drive, Carlisle, Pennsylvania 17013.
BEING the same premises which Gerald J. Goodhart and Nancy F.
Goodhart, husband and wife, by deed dated May 7, 1986, and recorded
May 8, 1986, in the Office of the Recorder of Deeds in and for
Cumberland County at Carlisle, Pennsylvania, in Deed Book "W",
Volume 31, Page 383, granted and conveyed to Roy E. Dippery, Jr.,
and Linda E. Dippery, husband and wife, Grantors herein.
