PETITION FOR CHANGE OF NAME
NOW COMES Your Petitioner, CHRISTINE L. KETROW, Natural Mother
of NICHOLAS CANAAN KETROW, by her attorney, Charles E. Petrie, and
respectfully represents as follows:
1. That Your Petitioner is CHRISTINE L. KETROW, Natural Mother of
NICHOLAS CANAAN KETROW, a minor child born on June 14, 1989, and
currently residing at 912 Bosler Avenue, Lemoyne, County of Cumberland,
Pennsylvania.
2. That Your Petitioner desires to change said minor child's name to
NICHOLAS CANAAN LITRENTA so that the child's last name will be the same
as his paternal grandfather, Dr. David E. Litrenta, with whom he has a very
close relationship. His name at birth was NICHOLAS CANAAN KETROW.
3. That the said minor child's natural father's name is Kevin T. Latrenta.
The natural father resides at 122 West Ninth Street, Suite 3, Cincinnati, Ohio.
4. That Your Petitioner has resided at the following addresses during the
past five years: from October, 1997, until the present at 912 Bosler Avenue,
Lemoyne, Pennsylvania.
WHEREFORE, Your Petitioner respectfully requests that Your Honorable
Court pursuant to 54 Pa.C.S. § 701, to:
(a) fix a hearing date for this Petition; and
(b) direct that notice be given of the filing of this Petition and of the
hearing.
