C.S. 6 701. et. seQ. The Petitioner, GAY LYNN WILDA Y, through her undersigned counsel, requests that this Court order a change of name for her, pursuant to 54 Pa. C.S. ~ 701, et. seq., and in support thereof, represents as follows: 1. Petitioner is GAY LYNN WILDA Y, a resident of 804 Fourth Street, New Cumberland, Pennsylvania, and has resided at the following location during the five years next preceding the filing of this Petition: 804 Fourth Street, New Cumberland, Pennsylvania, from 1989 to present. 2. Petitioner was born in Olean, New York, on April 24, 1971. 3. It is Petitioner's desire and intention to change her name from Gay Lynn Wilday to Gabrielle Lynn Wilday. 4, Petitioner has experienced that her name, Gay, is no longer acceptable as a formal name, that she has been embarrassed by her first name, and that she has endeavored to avoid it at all times, 5. A photocopy of Petitioner's birth certificate is marked Exhibit "A," attached hereto and made part hereof by reference. 6. Petitioner is a single person, has never been married and has no children. .. ""- - '.. .,." 7. Petitioner's Social Security Numberis 108-66-8148. A photocopy of Petitioner's Social Security card is marked Exhibit "B," attached hereto and made part hereof by reference. 8. There are no judgments or decrees oflike character of record against the Petitioner in Cumberland County, Pennsylvania, or any other state or county. 9. Pursuant to 54 Pa. C. S, ~ 702 (B) (1), attached is a copy of Petitioner's fingerprints that must be forwarded, along with a copy of this Petition, to the Pennsylvania State Police. Marked as Exhibit "C," attached hereto and made part hereof by reference. 1 O. Petitioner respectfully requests that the Court follow the newly adopted procedure and avoid the need for a hearing on the Petition; however, a proposed Order for Publication is attached for the convenience ofthe Court. 11.
