PETITION FOR NAME CHANGE
AND NOW comes Petitioner, Amy Giambilis, by and through her attorneys, Gross, McGinley,
LaBarre & Eaton, and respectfully requests that this Honorable Court order a change of name for Angela
Theresa Noga, and in support thereof, represents as follows:
1. Your Petitioner, Amy Giambilis, is the maternal grandmother and legal guardian of Angela
Theresa Noga, a minor. (A true and correct copy of the Court Order is attached hereto as Exhibit "A").
2. Petitioner and the minor child presently reside at 7 Graham Road, Newville, Cumberland
County, Pennsylvania.
3. Petitioner requests the court to change the name of Angela Theresa Noga, minor child.
4. Angela Theresa Noga's date of birth is August 2, 1994.
5. Angela Theresa Noga's social security number is 209-74-4663.
6. Petitioner, maternal grandmother and guardian of the minor, desires the change of name for
the following reasons:

a. Angela Theresa Noga has resided with, and has been cared for by, her maternal
grandmother, the Petitioner, since Angela was approximately six months old.
b. Angela Theresa Noga's biological mother, Kelly Giambilis, consents to the name
change and believes it to be in the best interests of the child. (Kelly Giambilis has executed a consent for
the name change; attached hereto as Exhibit "B").
c. Angela Theresa Noga's biological father, James Noga, consents to the name change
and believes it to be in the best interests of the child. (James Noga has executed a consent for the name
change; attached hereto as Exhibit "C".)
d. It is anticipated that Angela TheresaNoga will continue to reside with Petitioner until
she is oflegal age, or longer.
e. Angela Theresa Nogacurrently attends kindergarten, and Petitioner believes it would
be in the best interest of the child to have the same last name as the family with whom she resides.
7. Angela Theresa Noga is a citizen of the United States.
8. There are no judgments or decrees pending against Angela Theresa Noga.
