AND NOW, comes the Petitioner, Becky Lynn Weidman, by her
attorney, John J. Baranski, Jr., and represents the following:
1. The Petitioner is Becky Lynn Weidman, who resides at
942 Forest Court, Carlisle, Cumberland County, Pennsylvania 17013.
2. The Petitioner is a twenty four (24) year old adult
female, whose date of birth is May 21, 1975.
3. The petitioner has maintained follow residence over the
last five years during the dates indicated:
a. 942 Forest Court, Carlisle, Cumberland County, PA
17013, April, 1999 to the present;
b. Dickinson College, HUB 2230, Carlisle, Cumberland
County, PA 17013, September of 1993 to January, 2000;
c. 105 Eagle Peak Road, Robesonia, Berks County, PA
19551, August of 1998 to April of 1999;
d. 28 East Main Ave., P.O. Box 363, Myerstown,
Lebanon County, PA 17067, November of 1997 to August of 1998.
e. 7 East Park Ave., Myerstown, Lebanon County, PA
17067, March 3, 1995 to November of 1997,
4. The natural father of petitioner is Gene Weidman whose
last known address is 7 North Sheridan Road, Newmantown, PA 17073.
5. The natural mother of Petitioner is Mary Faye Anderson
whose last known address is 105 Eagles Peak Road, Robesonia, PA
19551.

6. Mother resumed her maiden name of Klotunowitch in 1993
until she remarried in 1997 to Ernest W. Anderson.
7. Gene Weidman and Mary Faye Anderson have been divorced
since September, 1993
8. That last contact Petitioner has had with Gene Weidman
was in November of 1995.
9. petitioner wishes to adopt the name Becky Lynn
Koltunovitch for the following reasons:
A. petitioner has had no contact with her natural
father for the last 8 and one half years and intends to have
no further contact with her father.
B. Peti tioner' s mother resumed using the surname
Klotunowitch immediately following her divorce from
Petitioner's Father until she remarried in 1997.
C. The surname Koltunovitch reflects the correct
spelling of her family surname and reflects what Petitioner
feels .LS her.. Lrue identity and she does not wish to be
associated with her father's surname.
10. There are no judgments or decrees of like character of
record against Becky Lynn Weidman in Cumberland, Berks or Lebanon
Counties.
WHEREFORE, Petitioner requests this Honorable Court, pursuant to
Act of December 16, 1982, P.L. 1309, No. 295, Section 2, 54 Pa.
Cons. Stat. ~ 701 et seq. to do the following:
A. Fix a hearing on the Petition;
B. Direct that notice be given of. this Petition; and
