Wesley Oler, Jr. on August 18, 2000 to support Petitioner's request for a name change to CONOR DAIGH as follows: 1. Petitioner was a resident of 1465 Hooksett Road, #424, Hookset, Merrimac County, New Hampshire 03106 on or about 1996-1997. By letter dated August 23,2000 counsel for Petitioner requested, and by information of the Hooksett District Court dated August 28, 2000, it was confirmed that no records of judgments or decrees oflike kind were found for Petitioner. See, Appendix A attached hereto and incorporated herein by reference. 2. Petitioner was a resident of 181 East Avenue, Apt. 303, Manchester, Nashua County, New Hampshire 03104 in 1996. On August 23, 2000 counsel for Petitioner requested the form for a records search from Nashua District Court, which form was received on August 28, 2000. Information was submitted by counsel on August 28, 2000. By information of the Nashua District Court dated September 5,2000, it was confirmed that no records of judgments or decrees oflike kind were found for Petitioner. See, Appendix B attached hereto and incorporated herein by reference. 3. Petitioner was a resident of 9 Quaker Lake Terrace, Orchard Park, Erie County, New York 14127 in 1996. By letter dated August 23, 2000 counsel for Petitioner requested, and by information of the Erie County Clerk's Office dated August 28, 2000, it was confirmed that no records of judgments or decrees of like kind were found for , , I. Petitioner. See, Appendix C attached hereto and incorporated herein by reference. 4. Petitioner was a resident of 10 Judith E. Lane, Methuen, Essex County, Massachusetts 01844 in 1995. By telephonic request of counsel for Petitioner on August 23, 2000 and by inforination of the Clerk of Court for Essex District Court on that date, it was confirmed that no records of judgments or decrees oflike kind were found for Petitioner. 5. Petitioner has been a resident of? West Mulberry Hill Road, Carlisle, Cumberland County for approximately the past two years. Counsel for Petitioner personally conducted a search of all records in the Cumberland County Courthouse for records of judgments or decrees oflike kind and found that there were none for Petitioner.
