of the attached petition of Julie L. Greenawalt, the guardian and natural mother of
Cody Ryan Henry, a minor, born May 5, 1992, for change of his name, and upon
presentation of this Petition to the Court, and it appearing that there is no lawful
objection to the request of the petitioner and the natural father having waived
further notice and without him being personally present when this Order is
entered, it is hereby ordered and decreed that the attached petition be granted,
without further notice to the natural father, Michael Blair, changing the name of
Cody Ryan Henry to Cody Ryan Greenawalt, which will be in the best interest of
the child.

In Re: Cody Ryan Henry,
PETITION FOR NAME CHANGE
1. Petitioner is Julie L Greenawalt, the guardian and natural mother of
Cody Ryan Henry, a seven year old minor who was born on May 5, 1992.
2. The petitioner desires to change her child's name from Cody Ryan
Henry to Cody Ryan Greenawalt.
3. The petitioner's reasons for desiring this change of name of her child
are that:
(a) The petitioner is not married to the child's natural father, Michael
Blair, of 844 Summit Avenue, Hagerstown, Washington County, Maryland.
(b) The petitioner desires to protect her child from the psychological
and emotional confusion which may result from the child and his parents having
different surnames and the change will be in the child's best interest.
(c) The child's natural father, Michael Blair, consents to this name
change.
(d) The residences of the petitioner during the five years preceding
the filing of this petition have been with the natural mother and for the last three
years also with her husband, Chad Michael Greenawalt in Cumberland County,
Pennsylvania.
4. Petitioner requests this Court:
(a) waive a hearing on this petition; and.
(b) direct that no notice be required of the filing of this petition and the
hearing.

5. Petitioner further requests this Court to waive the requirement for
official searches of the proper offices of the county wherein petitioner resides on
the grounds that the subject of this petition is a minor and avers that there are no
judgments or decrees of record or any other matter of like character against him.
6. The natural father of the child consents to the change of his son's
name.
7. The natural father further consents to waive a hearing, further notice, or
any requirement that he be personally present when this petition is presented to
the Court with an Order for signing a name change of his son.
8. Petitioner prays the Court to change her son's name from Cody Ryan
Henry to Cody Ryan Greenawalt.

and without my being personally present when the Order is entered by the Court
