services were rendered pursuant to applicable rate resolutions and ordinances,

rendered and the prices charged therefor, said statement being a true and correct
copy of the Customer Billing History File for Owner's account with South
Middleton Township Municipal Authority, which sum of $ 763.78 for the above
period was duly assessed against the above Owner/s and premises beginning

September 15, 1996 and for which sum, with interest, costs and penalties, a lien is
claimed against the premises in accordance with the Municipal Authorities Act of
1945, May 2, 1945 P.L. 382, 53 P.S, SS 301 et seq., as supplemented and amended
and the Municipal Claims and Tax Liens Act, May 16, 1923, P.L. 207, 53 P,S. SS
7101 et seq., as supplemented and amended,
