The Appellant is Sani.ay Pravin Patel, Vvhose current address is 2845 Lebanon Road, Manheim, Lancaster County, Pennsylvania 175450 2. The Appellee is the Commonwealth of Pennsylvania, Department of TranspDrtation, Bureau of Driver Licensing, having its principal place of business is the Office of Chief Counsel, Riverfront Office Center, 3rd Floor, Front Street, Harrisburg, Dauphin County, Commonwealth of Pennsylvania 17104. 30 On Saturday, May 27, 2000, at approximately 2:28 aom. the Appellant was operating a motor vehicLe, traveling south on UoSo Route 15 (near the Pennsylvapia Turnpike) in Upper Allen Township, Cumberland County, Commonwealth of Pennsylvania. 4. At that time and .place he was stopped by Township Police Officer James R. Subsequently, by letter dated June 13, 2000, the Commonwealth of Pennsylvania Department of Transportation, Bureau of Driver Licensing, the Appellant was informed that his operator's privileges are being suspended for a period of one (1) year, commencing on or about July 18,2000 for refusal to submit to a chemical test That letter was the Appellanfs initial notification of that chargeo A copy of that letter is attached, properly marked and identified as Exhibit' A.' 6. Pursuant to Appellant's rights under the Vehicle Code andJor other applicable statutes, he is appealing the proposed suspension of his operator's privileges to This Honorable Court, contendingJhat no such refusal occurred. 7. The Appellanfs operator's license number is set forth in the Criminal Complaint and in the Notice of Violation I,etter as 23420544.
